For identity verification, if you submit proof of identity and a selfie, we may share them with our identity verification partner. They may create biometric information for verification, which is not shared with MEDANA and is deleted by the partner after verification. MEDANA may receive extracted information (e.g., from your driver’s license) and a “match” confidence score to verify your identity.
- Purchase History (Information Specific to the Services): Information about goods or services purchased or obtained.
- Purpose: Processing/fulfilling orders and transactions, marketing, customer service, or analytics.
- Health and Medical Information (Identifiers/Sensitive Personal Information): Medical history and other information, symptoms, prescription history, insurance policy, and insurance eligibility and coverage. This may be sensitive personal information.
- Purpose: Processing and fulfilling orders and transactions. Such information will only be released when instructed by one of our patients.
- Financial/Payment Information: Credit card details and other payment information.
